Neuro_MR_IAC: 68 F, right CPA mass.

MRI BRAIN WITH AND WITHOUT CONTRAST:
 
CLINICAL HISTORY: 68-year-old female with history of disequilibrium.
 
FINDINGS:
 
Parenchyma: No acute hemorrhage, infarction, mass, or abnormal enhancement.
 
Ventricles and extra-axial spaces: Appropriate for age.
 
Orbits: Normal.
 
Visualized paranasal sinuses: Clear.
 
Mastoid air cells: Clear.
 
Bones: There is a 11 x 8 mm mass arising from the right temporal bone in a retrolabyrinthine location projecting into the right cerebellopontine angle. The mass is heterogeneously intrinsically T1 hyperintense with areas of enhancement. There is the suggestion of increased blood flow to the lesion on ASL perfusion images (series 7, image 8). There is also T1 hyperintense signal within the right vestibule and semicircular canals, which may represent intralabyrinthine hemorrhage.

IMPRESSION:
 
1.  11 x 8 mm right temporal bone retrolabyrinthine enhancing mass with associated increased blood flow, most likely endolymphatic sac tumor. There is intrinsic high T1 signal within the right vestibule and semicircular canals, suggestive of intralabyrinthine extension versus hemorrhage. Consider further imaging with MRI of the internal auditory canals and dedicated high-resolution CT of the temporal bone.
