Neuro_CT_Head: 23 F, headache and neck stiffness, hyperdense posterior fossa mass.

CLINICAL HISTORY: 23-year-old female with headache and neck stiffness.
 
FINDINGS:
 
Spanning the cervico-medullary and pontine-medullary region anteriorly, there is a 3.9 x 3 cm largely extra-axial high-density lesion without adjacent clival or other bony abnormality. This lesion results in mild cord compression and likely minimal downward cerebellar tonsillar herniation.
 
There is no acute large vessel territory infarction or hemorrhage. Gray-white matter differentiation of the brain parenchyma is preserved. Ventricles and remaining extra-axial spaces are appropriate in size and relatively symmetric in configuration. Visualized portions of the paranasal sinuses and mastoid air cells are clear. Bony orbits and their contents are unremarkable. No discrete calvarial defects are present.

IMPRESSION:

A 3.9 x 3 cm high-density, largely extra-axial lesion is present anterior to the pons and medulla. The inferior border is incompletely imaged. The differential includes extra-axial tumors versus complex cystic lesions. A contrast-enhanced MRI of the cervical spine and brain is recommended for further evaluation.
