15 yo F presenting with sinus pain and fever

Findings / Diagnosis

1. Pott's puffy tumor: T2 bright/T1 isointense material completely filling the left maxillary sinus and left frontal sinus. Additional scattered ethmoid air cell disease. Associated osteolysis of the inner table adjacent to the left frontal sinus with extension of presumed infectious material into epidural/subdural space. Latter is concerning for epidural abscess versus subdural empyema. Associated thickening and enhancement of the bilateral frontal dura (L > R).
