Neuro_MR_Brain: 2 M, GAS infection and multiorgan failure.

MRI BRAIN, ANGIOGRAM NECK WITHOUT CONTRAST:

CLINICAL HISTORY: 2-year-old male with sepsis secondary to Group A streptococcus infection.

FINDINGS:

BRAIN:

Parenchyma: Interval decrease in degree of restricted diffusion in the bifrontal periventricular white matter and bilateral cerebellar white matter, with stable associated T2/FLAIR abnormality. Extensive foci of susceptibility in the cerebral white matter, internal capsule, cerebellum, and corpus callosum. Stable extent of disease.

Ventricles and extra-axial spaces: Appropriate for age.

Orbits: Normal.

Visualized paranasal sinuses: Fluid in the bilateral maxillary and ethmoid sinuses.

Mastoid air cells: Bilateral mastoid effusions.

Bones: Normal.

Additional comment: None.

INTRACRANIAL ANGIOGRAM:

Anterior circulation: No flow-limiting stenosis or aneurysm.

Posterior circulation: No flow-limiting stenosis or aneurysm.

Additional comment: None.

EXTRACRANIAL ANGIOGRAM:

Motion limited.

Proximal great vessels: No flow-limiting stenosis or dissection.

Cervical vessels: No flow-limiting stenosis or dissection.

Additional comment: None.

PERFUSION: Normal.

 IMPRESSION:

1.  Evolving findings compatible with hemorrhagic leukoencephalitis, not significantly changed in extent.

2.  No flow-limiting stenosis, occlusion, or aneurysm in the intracranial and extracranial circulations.

(Hemorrhagic leukoencephalitis)
