Neuro_MR_Brain: 10 F, AMS.

MRI BRAIN WITH AND WITHOUT CONTRAST:
 
CLINICAL HISTORY: 8 years of age, female, posterior fossa tumor.
 
FINDINGS: 
 
BRAIN: 
 
There is a lobulated, T2 hyperintense mass centered on the fourth ventricle extending into the subarachnoid space through the foramina of Luschka and Magendie, and expanding the obex, with small cysts in the superior aspect. Inferior aspect of mass extends to the C1-C2 level, and extends laterally into the left greater than right premedullary cisterns encasing multiple vessels including left PICA. Mass demonstrates patchy areas of enhancement postcontrast without reduced diffusivity. Mass measures 5.1 x 4.0 x 4.9 cm (AP by ML by CC). Mass results in obstructive hydrocephalus of the fourth ventricle with resultant moderate dilatation of the lateral and third ventricles with periventricular and optic pathway edema. No additional lesions seen in the brain. No evidence of leptomeningeal spread. 
 
IMPRESSION: 
 
1.  Fourth ventricular mass extending into the foramina of Luschka and Magendie, with resultant ventriculomegaly and tumor surrounding/deforming the medulla and the upper cord.  Findings likely represent ependymoma rather than other tumor types.
