Neuro_CT_Head: 23 M, head trauma.

HISTORY:  23-year-old male, head trauma.
 
HEAD CT:

There is a large left frontal-temporal-parietal epidural hematoma that is heterogeneous in attenuation, likely 
reflective of hyperacute on acute bleeding. Scattered foci of air 
are seen. This hematoma measures approximately 13 cm AP x 4 cm transverse and is associated with significant mass effect on the 
left hemisphere, right midline shift of approximately 17 mm, uncal 
herniation, and downward transtentorial herniation, with effacement of the suprasellar cistern. There is local sulcal effacement throughout the left cerebral hemisphere.
Mass effect upon the midbrain is also present. There is early trapping of the right lateral ventricle. There is an associated nondisplaced fracture of the squamosal left temporal bone as well as the mastoid left temporal bone, which extends into the left middle ear cavity. Associated small amount of fluid and hemorrhage is seen within the left mastoid air cells and middle ear cavityThe fracture line extends vertically along the left parietal bone.
 
CERVICAL SPINE:  There is no prevertebral soft tissue abnormality.  
Nasogastric and endotracheal tubes are partially imaged. No fracture or malalignment is seen.
 
IMPRESSION: 
1. Large left hyperacute on acute frontal-temporal-parietal epidural hematoma with significant mass effect, including rightward subfalcine herniation, uncal herniation, and downward transtentorial herniation with effacement of the suprasellar cistern. Mass effect on the midbrain is also present.
 
2. Associated nondisplaced fracture involving the squamosal left temporal bone which extends vertically along the left parietal bone.

3. Additional nondisplaced fracture involving the left temporal mastoids, extending into the left middle ear cavity, with associated hemotympanum.

4. No fracture or malalignment of the cervical spine.
