MRI BRAIN WITH AND WITHOUT CONTRAST:

CLINICAL HISTORY: 64-year-old woman with ataxia.

FINDINGS:

Exam is significantly degraded by motion artifact.

Parenchyma: There is increased diffusion signal in the right greater than left caudate heads, right caudate body, and bilateral anterior putamina, as well as prominent cortical diffusion signal. ADC limited due to marked motion degraded degradation on B0 images. There is likely associated increased T2/FLAIR signal in the anterior basal ganglia and cortex, though FLAIR images significantly limited by motion. No definite abnormal enhancement postcontrast. No evidence of hemorrhage. No significant volume loss at this time.

Ventricles and extra-axial spaces: Appropriate for age.

Orbits: Normal.

Visualized paranasal sinuses: Clear.

Mastoid air cells: Clear.

Bones: Normal.

Additional comment: None.

IMPRESSION:

1. Significantly motion degraded exam. T2/FLAIR and diffusion signal abnormality in the right greater than left caudate nucleus and anterior putamen, as well as bilateral cerebral cortex, right greater than left. No significant volume loss at this time. In spite of motion degradation, findings raise possibility of Prion disease.
